When completing this application form, please refer to the online guide entitled "How to Apply for Funding - A Guide to Preparing a Proposal", which provides detailed information on all of the points that you are required to address in your funding application. Please also refer to the attached Annex A, which describes the theme for NVCAW 2010. How the theme is reflected in your proposed project is one of the most important aspects that will be considered in the selection process. This form, once completed, contains all the necessary information to allow us to fully assess the merits of your proposal for NVCAW 2010. Due to the growing number of applications, the deadline for submitting applications has been advanced this year. The new deadline to submit your funding application is November 1, 2009. We regret any inconvenience this change may cause. Applications will only be accepted up to this date. As funding is limited each year, not all eligible applications can be approved.

All eight sections must be completed. Use the space you need and be sure to address each point separately. Please consult the online Guide for an explanation of the information you are being asked to provide.
- Describe your project.
The description/summary should provide a brief explanation of what the project is, it should describe the activities/events you propose to hold during the Week, who will be affected by the activities and why you want to undertake the project (e.g. expected outcomes; what you want to change; etc.).
- Indicate the interest in your community for this project, as well as the partnerships and other linkages you have secured indicating support for this project. (Please include letters of support IF you have them).
- The project's objectives.
The objectives must be results-based, measurable and achievable. You should identify what you expect to achieve by the end of the project. Your objectives must also link to the overall theme of the NVCAW 2010: "Every Victim Matters" (refer to Annex A for details).
- How the project demonstrates sensitivity to diversity and gender equality issues (e.g. how the needs of people who come from different social and cultural groups, equity groups and/or diverse communities have been taken into consideration).
- How the project demonstrates sensitivity to the needs of Official Language Minority Communities (e.g. how the needs of people from these communities {i.e. Francophones who live outside Québec and Anglophones who live in Québec} will be addressed, as applicable).
- The work plan and project design (i.e. Project Activities) (Include distinct activities and how each activity will support the project objectives, tasks for each activity, time frames, etc.).
- The anticipated results of the project. Please also include estimated project result indicators (e.g. number of participants, number of printed/distributed materials, etc.). Note: For each project objective that you identified above, you should identify the results that are expected as a consequence of your project's activities.
- The project deliverables and dates for submission to the Department of Justice Canada (i.e. Project Outputs). Be sure to include a Project Summary Report*, which should consist of an executive summary of the project, an overview of the project activities, and any documents that were produced/distributed as part of the project, at a minimum. * We will provide you with a template to follow for this report, if your application for funding is approved.
Partnerships

Evaluation plan
Describe how you intend to measure the success of your NVCAW 2010 event/project.
The evaluation should be "short and sweet". For example, a participant survey, the number of attendees at your event, number of pamphlets that were produced and distributed, the number/frequency of newspaper or radio ads, etc.
Budget
Indicate the budget for your project - up to a maximum of $10,000 in revenue from the Department of Justice Canada. Be sure to include detailed information on the costs, along with a breakdown by line item. Please refer to the attached Annex B for a sample budget. You can also consult our online "FAQ" for additional information and a list of eligible and ineligible expenses.
Include all revenues (financial and in-kind from ALL sources) and all expenses (financial and in-kind) by fiscal year (e.g. April 1, 2009 to March 31, 2010). The federal government fiscal year starts on April 1 of a given year and ends on March 31 the following year - therefore all expenditures must be expended within that fiscal year.
Please indicate your organization's GST/HST rebate (percentage) from the Canada Revenue Agency.
Note: Funding for National Victims of Crime Awareness Week 2010 projects will be available up to March 31, 2010 for planning and organizing activities prior to the Week itself, as well as from April 1, 2010 to May 7, 2010 for hosting/supporting an event during the Week. In other words, funding that spans two fiscal years is being made available in order to give applicants some flexibility in planning/organizing prior to hosting events during NVCAW. The maximum amount of funding that can be provided by the Department of Justice Canada for NVCAW 2010, whether or not it spans more than one fiscal year, is $10,000.
Conditions
Departmental funding may be used only for the purposes specified in this application. Once the Department has agreed to provide financial assistance, no substantial change to the project shall be made without the consent of the Department, and it shall be at the discretion of the Department to determine what constitutes substantial change in each case. Any funding not used for these purposes must be returned to the Department.
The organization is wholly responsible for its own debts. The Department will not consider any application to pay debts. If any part of this funding is used to pay salaries or honoraria, federal and provincial laws concerning salaries and source deductions must be applied (e.g. deductions for income tax, employment insurance, etc.).
Public acknowledgement of funding provided by the Department is required. The Department's financial assistance must be clearly acknowledged in any publications, written or print material produced as part of the project and/or during any public presentations. (e.g. We wish to acknowledge the funding that was provided by the Department of Justice Canada Victims Fund). The Department of Justice Canada logo is available for your use, as well as the NVCAW 2010 logo.
The organization agrees to respect and apply the spirit and, if applicable, the letter of the Canadian Human Rights Act and of the Official Languages Act.
In the event of an access to information request regarding the present funding application or any other information about the organization in the Department's possession, the organization will be consulted before any confidential information is disclosed. Under the Access to Information Act, there is provision for exemption from disclosure of personal information subject to the Privacy Act, and of confidential financial or business information. The designation PROTECTED (once this application form is completed) ensures that this information receives enhanced protection. When funding is approved, the amount of funding, the purpose for which the funds were granted and the name of the organization receiving the funding are considered public information.

Annex A
National Victims of Crime Awareness Week 2010
The 5th annual National Victims of Crime Awareness Week (NVCAW) will be commemorated across Canada from April 18 - 24, 2010. The theme that has been chosen for NVCAW 2010 is: Every Victim Matters.
This theme recognizes:
- That every victim of crime - no matter who they are, where they live or the way in which they were victimized - should be treated with compassion and respect for their dignity.
- That victims of crime have a role in Canada's justice system and there are laws, policies and programs which give them an opportunity to participate and have their voices heard at different stages in the criminal justice process.
- The dedication of professionals and volunteers alike who work tirelessly and selflessly with victims of crime and their families and who show, in their daily actions, that every victim truly does matter.

Some examples of eligible events and activities for NVCAW include:
- Workshop or information sessions on victim issues or relevant topics;
- Public awareness events (e.g. BBQ; community walk; information kiosk; etc.);
- Production of promotional items for your organization for use during NVCAW;
- Public information flyers/brochures/pamphlets on victim issues and/or available services to victims and their families;
- Training for justice system professionals on victim-related issues and/or needs;
- Special issue of a newsletter dedicated to NVCAW; and,
- Radio, newspaper or television ads promoting NVCAW and/or providing information on victim services.
